While we encourage online response to the NTEWS, some participants have been selected to complete the NTEWS paper questionnaire.
For those who receive the paper questionnaire, please complete it using a pen with blue or black ink, or a #2 pencil. Mail it back in the postage-paid return envelope. If you have trouble answering a question, call us at 1-888-329-5903 or email us at ntews@census.gov, and we will help you.
You do not need to return the paper questionnaire if you complete the survey online or by telephone.
If you have lost the return envelope, send your completed questionnaire to:
