Information for Respondents

Respond to the Survey

If you received a letter from the U.S. Census Bureau requesting your response to the National Survey of Children’s Health – Longitudinal Cohort, learn more about responding and why each completed survey is important. 

Gather the materials we mailed to you.

1.       Go to https://respond.census.gov/nschlc

2.       Enter your 8-digit Login ID provided with your mailed materials. 

Even if there are no children or young adults ages 3-24 years old that have lived with you or another adult in the household since 2018, please log in to let us know.

3.       After successfully logging in, you will be assigned a 4-digit PIN and asked to select a security question.

Make note of your assigned 4-digit PIN. The PIN is required every time you enter the survey. If you forget this PIN, you will be asked to provide your security question response to recover it.

Is it safe to provide my information over the internet?

Your information is always encrypted. Our secure servers use "HTTPS," Hypertext Transfer Protocol over Secure Socket Layer, to ensure the encrypted transmission of data between your browser and the U.S. Census Bureau.

This means that instead of sending readable text over the Internet, both your browser and our server encode (scramble) all data using a security key. In addition to data being encrypted during transmission, all data are encrypted for protection when stored on the Census Bureau servers.

How do I recover my Web survey Login ID or PIN?

If you have lost or misplaced your Web Login ID and/or PIN, please call the U.S. Census Bureau toll-free at 1-877-749-4943 for assistance. For Telephone Device for the Deaf (TDD) assistance, please call: 1-800-582-8330.

You can respond in one of the following ways:

  • Web questionnaire
  • Paper questionnaire

As noted in the letter you received requesting your participation, if you cannot complete the survey by Web, you may contact the U.S. Census Bureau at 1-877-749-4943 or ADDP.NSCH.LC@census.gov for assistance. You can also contact us if you received a Web questionnaire invitation only and prefer to respond by paper.

Your household is unique and important. We need to hear from households from all walks of life to collect enough information to help improve the health of children and young adults across the country. If a household that has been selected for the survey does not respond, we cannot replace them with another household.

The information that you provide is used solely for statistical purposes. All U.S. Census Bureau employees take an oath of confidentiality and are subject to fines or imprisonment for improperly disclosing information. Names and addresses are removed from all forms, and are not included in any statistical release. Also, the NSCH-LC NEVER asks for:

  • your Social Security number
  • your personal information via email
  • money or donations
  • credit card information
