Please let the Census Bureau know by taking 5 minutes to fill out the form below. For any fields that are not relevant to your incident, please leave them blank. If you include your email, the Census Bureau’s Trust & Safety Team may follow up with additional questions as necessary.

The purpose of collecting this information is to provide a mechanism for the public to submit suspected misinformation, disinformation, scams and fraud to the Census Bureau’s Trust & Safety Team. Personally identifiable information collected is protected by the Privacy Act of 1974 (5 U.S.C. § 552a) and includes email address.

Disclosure of the information provided to us is permitted under the Privacy Act of 1974 (5 U.S.C. § 552a) and may be shared with other Census Bureau staff for the work-related purposes. Furnishing this information is Voluntary. There are no consequences for not providing information. The data is collected from census.gov and stored on a secure server. The Census's Trust & Safety Team will have access to the information.
